HEALTH QUALITY INITIATIVE
North
Dakota Health Care Review, Inc. (NDHCRI), as the Centers for Medicare
& Medicaid Services (CMS)-designated Medicare Quality Improvement
Organization (QIO) for North Dakota, is committed to working with North
Dakota’s home health agencies to improve their performance on Home
Health Compare publicly
reported measures. Home health agencies are working to reduce avoidable
hospitalizations, and either improve patients' ability to manage oral
medications or reduce pain that interferes with activity. The
home health agencies have incorporated influenza and pneumococcal
immunizations into a
comprehensive patient assessment. A subset of home health agencies have
implemented phone monitoring or telemonitoring to help
reduce acute care hospitalizations.
Home health agencies that participate with NDHCRI have the opportunity
to:

up-to-date target outcome data analysis
* Network
with peers to share best practices and lessons learned
This
national home health quality improvement campaign is sponsored by the
Centers for Medicare & Medicaid Services (CMS), and targets all
home health agencies and key home health stakeholders in the nation.
The goal of this national campaign is to unite the home care community
under the shared vision of reducing avoidable hospitalizations to
improve patient quality of care. Home health agencies enrolled in the
campaign receive tools and resources, guidelines, information and
best-practice education.

NDHCRI will continue to support each HHA's quality
improvement efforts regardless of whether or not agencies enroll in the
national campaign.
